About Continuum Care

Continuum Care (Cornwall) strives to empower people to live to their optimal capacity in their own homes. We support people in a positive, non-judgmental way, honouring their lives and respecting their individuality. Where there are challenges, we hold firm that it is the condition, not the person, that presents the challenge. By working supportively alongside our clients and their families, we help create a safe framework for people to live with dignity and purpose.

Continuum Care is listed as one of the preferred providers in Cornwall for both traditional at-home care and supportive lifestyle care.

Our approach

Continuum Care (Cornwall) prides itself in supporting people to remain in their own homes for as long as is practicable and safe for them to do so. We build our care and support packages through the following framework:

  • Listening – enabling clients to tell their stories in their own words
  • Understanding – receiving clients and their stories with insight and empathy
  • Responding – offering support that is timely, holistic, developmental, non-blaming and tailored to the individual’s needs
  • Person-centred – the person is not their condition, and if challenging, these challenges come from the condition and not the person
  • Monitoring – ongoing observation and review of each individual’s progress, to ensure our processes and services remain meaningful, appropriate and effective in building beneficial outcomes

Our services

Continuum Care currently provides the following care and support services in Cornwall:

Adult Domiciliary Care

We support people of all ages, and with a wide range of mental health and physical conditions, to remain in their own homes for as long as they wish or as long as is safe to do so.

Adult’s Supportive Lifestyles

Working with clients to maintain, regain or learn new life skills that help them towards optimal levels of independence, security of tenure and integration within their community.

Children’s Services

Under application: small homes aimed at young people with Autism, Asperger’s and / or physical and mental health challenges.

Outreach Services

In development: support for adults and children in their own homes to overcome difficulty in specific areas of their lives, e.g. education, training, life skills and community integration.

Facilitation Space

To be developed: rooms in our premises where families can meet for support, discussion and signposting towards relevant services.

Forest School

In research: a piece of land where all our clients can participate in education and learning in an inclusive, alternative outdoor environment.

Our team

Continuum Care is led by a team of highly experienced care professionals with backgrounds in education, volunteering, adult residential and children’s homes, care for people with mental health and physical disabilities, law enforcement, the charity sector, community care and life coaching.

On top of the relevant training and experience we look for the interpersonal qualities that reflect our values: the capacity for empathy, respect, understanding each client’s individual needs, valuing what matters to them, honouring their aspirations, and taking pride in their journeys towards greater wellbeing. Our Management Team currently comprises seven people with extensive professional and personal skills.

Commissioning our services

Continuum Care (Cornwall) is on the Cornwall-approved care provider list. We have been vetted and approved to deliver packages of care commissioned by the Local Health Authority and Cornwall Council on their behalf.

Some client families choose to seek services independently of the local authorities. Families are welcome, via independent financial means, to commission a service directly from us for their child or relative.

Origins

Continuum Care (Cornwall) is a sister company of First Call Care Solutions Ltd, an organisation founded in 2014 to provide training and management support to other care and support organisations throughout the Southwest of England.

In 2016 Continuum Care (Cornwall) was registered with the Care Quality Commission to provide domiciliary care services to people living in their own homes.

Continuum Care (Cornwall) prides itself in supporting people to remain in their own homes for as long as is practicable and safe for the individual to do so. The Agency is listed as one of the preferred providers in Cornwall of both traditional at-home care packages and supportive lifestyles care packages for individuals for the whole of life.
